Ever since the Sentinel was introduced in GTA III, a sportier variant has always been present in every GTA game, excluding GTA San Andreas. In GTA III and Liberty City Stories, only the Leone Family gang members drive this car, which is therefore known as the Mafia Sentinel (Leone Sentinel in Liberty City Stories).

Game system files of GTA Vice City, refer to the Sentinel XS as "Mafia" proving that the model was a copy based off the Mafia Sentinel. The Sentinel XS's main difference in comparison to the regular Sentinel include the spoiler with window grilles on the back of the car as well as being a bit faster. In GTA Vice City Stories, the Sentinel XS is the preferred gang car of the Mendez Cartel.

GTA IV features an updated Sentinel XS and the game refers to the car as a regular Sentinel when entered, however, the notable spoiler remains giving the car the name. It also has a rear spoiler sporting an "STD" badge. The STD supposedly stands for "Sentinel Tuning Division", but in real life the acronym commonly means "sexually transmitted disease". GTA IV features an updated Sentinel XS and the game refers to the car as a regular Sentinel when entered, however, features different badging, giving the car the XS name. Being just a variant of the Sentinel, it handles exactly the same as the standard Sentinal. It also has a rear spoiler sporting "STD" decals. STD stands for "Sentinel Tuning Division", but in real life the acronym commonly means "sexually transmitted disease". STD could also be a parody of Subaru's STi marque or a play on BMW's in-house tuning division, BMW M. The GTA IV rendition emulates the BMW M3 GTR.
