Well, I'm firmly against GTAs using past protagonists again (just cheap, lazy and wrong), and considering IV and the III era take place in presumably different canons, the III canon which had just been abandoned, revisiting the III era for V seems unlikely. And travelling between two large regions (D.C is too small for a whole city, but Virginia and Maryland additions would be great additions that wouldn't function as standalone games) would be extremely hard for the developers.
