Post game
In the final storyline mission, Johnny meets up with Thomas Stubbs in the The Lost MC clubhouse. Stubbs informs Johnny that Billy is about to turn state's evidence and testify to the FIB against The Brotherhood because he believes that it was Johnny that put him in prison. Johnny decides to take out Billy before he gets a chance to say anything. He heads to the Alderney State Correctional Facility, where he calls Clay and Terry for backup. Terry supplies Johnny and The Lost with infinite guns and ammo for free for the assault. Johnny blows up the main entrance and the trio starts clearing the area of guards. After killing about 50 guards, two helicopters arrive with NOOSE teams.
After the credits, Johnny finds himself outside Brian Jeremy's Safehouse. From this point on, the clubhouse is inaccessible and its weapon and vehicle spawn points disabled, but all are still active in this second safehouse. The parking spot in front of the safehouse can still be used for storing vehicles, however, and any left parked there before the endgame will still be there. Despite the comments made in the final cutscene about the demise of the club, in the post-game Johnny can still engage in gang warfare and request gun van service and bike delivery from his members.

After killing basically every guard in the prison, Johnny finds and executes Billy. Then they quickly get back to their bikes. When they reach the main entrance, that Johnny blew up, they witness prisoners escaping. Because of this, the police don't bother them when they head back to the clubhouse.

At the clubhouse, the remaining members of The Lost Brotherhood decide to burn it to the ground and ultimately break up the chapter for good. The gang empties the clubhouse and uses gasoline to start the fire. They all go outside and watch their clubhouse burn to the ground.

Notes
It is very important to watch the credits after the cutscene ends. As the credits roll, the camera pans around Liberty City, showing certain missions and things that happens during Niko's storyline and showing where Johnny is in the situation (i.e. Niko killing Vlad, then panning backwards to show Johnny and the Lost riding their bikes on a nearby street), along with a climax point during the museum heist mission which then pans out to show Luis Fernando Lopez, the protagonist of The Ballad of Gay Tony, on the roof. They are very fun to watch, full of easter eggs, and gives the player a nice look onto the whole storyline as a whole. It ends with the camera panning up on the McReary bank heist, where The Ballad of Gay Tony storyline begins.

Get Lost is a mission and the finale of Grand Theft Auto: The Lost and Damned. It is given by Thomas Stubbs to Johnny Klebitz but is for the Alderney Chapter of the The Lost Brotherhood .

By matching up the time lines it can be assumed this mission takes place around the same time Niko Bellic encounters Bernie Crane in Weekend at Florian's in the original game.

Mission Description

After escaping Ray Boccino and the death of Jim Fitzgerald, Johnny recieves a call from Congressman Tom Stubbs, asking Johnny to meet at the Lost MC Clubhouse.

Do not trigger the mission until you have completed any activities you wish to partake in at the clubhouse (including watching TV) and have completed all side-missions required for 100% completion, if you are aiming for the accomplishment. If you're low on armor or ammo, and you have no other missions to complete, don't waste your money at this point, for reasons soon to become obvious.

At the Clubhouse, Stubbs explains to Johnny that Ray Boccino and his associates will either be killed or arrested in the next few months; this proves to be true in Grand Theft Auto IV.

He also explains that Billy Grey is going to testify against Johnny and the rest of the Alderney Chapter of the Lost MC, pinning a major drug dealing operation on them. Billy then plans to enter the Witness Protection Program.

Johnny calls up the reamining Lost MC members and tells them to meet him at the Alderney State Correctional Facility and to "bring all the fucking firepower you got." Terry Thorpe, Clay Simons, and a few other Lost MC members join up with Johnny outside the prison. At this point Johnny can access Terry's gun van, but this time all weapons and armor and ammo are free, so stock up with the most powerful weaponry he's got.

Johnny blows up the front entrance and the Lost storm the prison, slaughtering their way through the prison yard. After a huge fire-fight with police, Johnny heads to the block where Billy is being held.

Billy is still outside, having killed a prison guard with a knife. Him and Johnny have some back and forth, Billy advances at Johnny with the knife and Johnny shoots him in the chest. Billy falls back, looking up at Johnny, until he finally dies.

Johnny and the Lost ride back to the clubehouse only to discover it is completely trashed by Ray Boccino's men. Johnny, (now joined by Angus Martin) and the remaining Lost members reflect on the life they led and the stupidity of it all, how the greed and drugs had changed so many friends. None of it for the better.

They agree to put the clubhouse out of it's misery. Terry douses the main room with gasoline, Johnny pushes Angus in his chair out to the street followed by Clay. The four stand on the street, staring up at the blazing inferno that was once their home, and prepare to take on an unexpected future.
