You can get into the Old Hospital's second floor by going around to the back and climbing a ladder. This method allows you to skip fighting most of the gangsters, and you can escape through the back with some tricky jumping. You must jump on an empty spool in the room to the right of where you climbed in, and then try to jump on the first story windowsill in front of you. From there, jump up to the second story windowsill above you (Niko will grab it) climb over the window and run for the dock, where there is always a boat waiting after you get the coke.
