:Can't you just change each one to link to the correct rendition? I know that's a lot of edits, but it only has to be done once, and you can easily work from [[Special:WhatLinksHere/Liberty City|this list]]. In terms of [[Liberty City]] to the most recent rendition, I would only do that if there was one page/rendition that was used a LOT more than any of the others. An example could be [[Mike]] - most people would be looking for the protagonist Mike, so that's where the page goes - the other (much less used) pages are linked to separately. But for Liberty City, both the GTA III Era and GTA IV Era pages are used a lot, so we can't really relegate one of them under the other - it would get confusing.
