Patrick "Packie" McReary (voiced by Ryan Johnston) is a brash, hot-tempered but generally amicable Irish-American thug who is the heir to Liberty City's declining Irish Mob. Youngest of the McReary family, he is younger brother to police detective Francis McReary, Derrick and Gerry. He lives with his mother, Mrs. McReary and elder sister, Kate. Niko first meets Patrick through Elizabeta Torres who is also his friend and sometimes business associate. Although Niko's first interaction with Patrick is done as a favor for Torres, he has various opportunities of his own that Niko can take advantage of later in the story.

Although a somewhat complex character, Patrick can be viewed as having somewhat stereotypically Irish traits, including an addictive personality, specifically to alcohol and narcotics such as cocaine. Many of his issues may be traced to his abusive father who has passed on prior to the events of the story. Patrick reveals that his father was a raging alcoholic who caused the McReary's fall from grace. He also implies that he may have been physically abusive to him and his brothers in more ways than one. Because of this, Patrick harbors a strong hatred for his deceased father, condemning his father's memory to Hell on numerous occasions.

Due to Patrick's criminal lifestyle, he has seen the inside of juvenile detentions and even prison in his life but never for any protracted length of time like his older brother, Gerry. It is implied however that Patrick may either have been victimized or the perpetrator of prison rape during his few times behind bars. This is alluded to in the "Luck of the Irish" mission during a conversation with Niko. Patrick also seems to be confused about his sexuality, at least when he is under the influence, as he also relates this to Niko during a conversation when going to any recreational activity. More than anything else, Patrick is portrayed as a tormented and even guilt ridden individual who feels more inclined to put on a wise-cracking facade rather than open up and relay his true feelings.

If Patrick's likes Niko enough, he will urge Niko to call on him whenever the man needs a car bomb planted.
