The Messina crime family is the third most-powerful and is one of the Italian-American families based in Liberty City, and part of the Cosa Nostra The Commission. The Messina syndicate is based in Algonquin and Dukes, Liberty City. The family is believed to be led by Harvey Noto, 66, who was able to establish himself at the top of the organization after a long and bloody power struggle. Mark Volpe was a Capo whilst Fred Volpe, another capo and brother of Mark, has been a police informant since 1998. Harry Hall is Consigliere of the Messina family. The family only appears in GTA IV in Deconstruction for Beginners; but they also make another on-screen appearence in Grand Theft Auto: The Ballad of Gay Tony in the mission Practice Swing where they attempt to kill Rocco, Luis, Tony and the Union Official over a labor union despute (likely the same dispute that forshadowed the mission Deconstruction for Beginners). In Grand Theft Auto: Chinatown Wars, Huang Lee is tasked with killing Messina member Jimmy Capra on the orders of Rudy D'Avanzo. It's named after former boss John Messina.

Notes
They are probably based on real life Bonanno crime family in New York City which is one of the five families. They share a similar history of bloody infighting and each received help from another family in the commission: The Messina's from the Gambetti's and the Bonanno's from the Gambetti's real-life counterparts, the Gambinos. The name of the family is also a reference of one of the old Dons of the Bonanno family, Joe Massino, while the first boss of the Messina family has a similar name; John Messina.

- The Messina's are heavily involved in the Construction Buisnesses as shown in mission "Practice Swing" when Rocco is trying to shakedown a union official when the official breaks, he says: "they're with the Messinas, you'll never be able to get in".
- The Messina's are the family who are trying to shakedown Yusuf Amir and the people who Niko Bellic kills in the mission "Deconstruction for Beginners".
- Harvey Noto boss of the Messina family owns the Majestic Hotel
